Celiac sprue Gluten enteropathy

Celiac sprue
  • Facts:
    Autoimmune malabsorption disorder of the small intestines
  • History / PE:
    Dermatitis herpetiformis
    Iron deficiency anemia (iron absorbed in duodenum)
    Intermittent abdominal distension
    Flatulence
    Greasy stools
  • Diagnosis:
    Anti-gliadin / anti-endomysium antibodies
    Effacement of small intestine villi
    Abnormal D-xylose absorption (no change after ABX)
    Iron deficiency anemia
  • Treatment:
    Gluten-free diet
  • Complications:
    Osteopenia, osteoporosis
  • Differential Diagnosis:
    1) Bacterial overgrowth (abnormal D-xylose absorption, normalizes post abx)

    2) Topical sprue (small intestine involvement, recent travel)
