Follicular thyroid carcinoma

  • Facts:
    Accounts for 15% of all thyroid cancer
    More malignant than papillary thyroid carcinomas
    Spreads hematogenouslsy, metastasize to distal organs
  • History / PE:
    Hard, fixed, nontender thyroid nodule
  • Diagnosis:
    Fine needle aspiration biopsy (FNAB) shows follicular cells
    Hurthle cells (also in papillary cancer)
  • Treatment:
    Near total thyroidectomy with post op radioiodine ablation
  • Differential Diagnosis:
    Follicular adenoma (no blood vessel invasion)
