Autosomal dominant disease, MC in african americans
Disease of the myocardium, where a portion gets hypertrophied without an identifiable cause (obstructive or non-obstructive)
Causes a primarily diastolic heart failure/dysfunction
History / PE:
Syncope Sudden death in a young, otherwise healthy, athlete
Crescendo-descesendo murmur at LL sternal border (valvalsva maneuver increases murmur intensity)
Diagnosis:
Carotid pulse with dual upstroke (midsystolic obstruction that develops as heart contracts)
Most murmurs decrease with decreasing preload, with the exception of the murmur for HCM which increases (decreased blood lessens the size of the ventricular cavity and causes increased outflow obstruction)
Associated With:
Systolic anterior motion (SAM), an abnormality in the motion of the mitral valve leaflets, which results in increased outflow obstruction
