Pheochromocytoma

Pheochromocytoma
  • Facts:
    Tumor in adrenal medulla or in extra-adrenal site that secretes catecholamines
  • History / PE:
    Excessive perspiration
    Uncontrolled HTN
    Tremors
    Abdominal pain
  • Diagnosis:
    24hr metanephrines
    MIBG scan may help
  • Treatment:
    Surgery
    Labetalol (has both alpha and beta blocking)
  • Notes:
    Alpha blockade first, then beta blockers to control tachycardia
    Unopposed alpha blockade will lead to refractory HTN

  • Associated With:
    vHL syndrome
    NF
    MEN IIa and MEN IIb
  • Differential Diagnosis:
    Hyperthyroidism
