  • Facts:
    MCC of cyanotic congenital heart disease
    See cyanosis within first year of life
  • History / PE:
    Pulmonary stenosis
    Overriding aorta
    VSD
    RVH
  • Diagnosis:
    Pansystolic murmur
  • Treatment:
    Surgery
  • Complications:
    "Hypercyanotic" or "tet" spells
  • Notes:
    Knee to chest maneuver relieves "tet" spells by increasing systemic vascular resistance, preventing blood from flowing through R->L VSD, instead forcing it through pulmonary artery
  • Differential Diagnosis:
    1) Hypoplastic left heart syndrome (heart failure within first few weeks of life)

    2) Transposition of the great vessels (cyanosis within 24hrs of life)
