Wolff-Parkinson-White syndrome
  • Facts:
    Transient arrhythmia via accessory pathway
    Commonly appears in early childhood
  • History / PE:
    Dizziness, dyspnea after exertion
    Occasional palpitation
  • Diagnosis:
    Look for delta wave
    Shortened PR interval
    Widening of QRS complex
  • Treatment:
    Procainamide or quinidine
  • Complications:
    Atrial fibrillation
  • Notes:
    Treatment involves conversion back to sinus rhythm (eg. cardioversion or antiarrhythmic), as opposed to drugs that would serve to increase AV refraction (eg. CCB, β blockers, digoxin , adenosine) because they would increase conduction via accessory pathway
  • Associated With:
    Atrioventricular reciprocating tachycardia
