Drug overdoses

  • Facts:
    Aspirin
    Tinnitus, hypogylcemia, vomiting
    Metabolic acidosis and a respiratory alkalosis
    Treat by alkalinization of urine with bicarbonate

    Tricyclic antidepresssants
    Treat with sodium bicarbonate

    Tylenol
    Treat with N-acetelycystein (mucomyst)

    Benzodiazepines
    Slurred speech, unsteady gait, normal pupil size
    Treat with flumazenil

    Opioid
    Respiratory depression, meiosis, sedation
    Treat with naloxone

    Lithium
    Tremor, hypereflexia, ataxia, seizures
    Treat with hemodialysis

    Phenytoin
    Horizontal nystagmus, cerebellar ataxia, confusion

    Beta blockers
    AV block, hypotension, diffuse wheezing, bradycardia
    Treat with atropine and IVF (glucagon as 2nd line)

    Digoxin
    Blurred vision, disturbed color preception, fatigue, HAs
    Bradycardia, AV block, hypotension, but NO wheezing!
    Treat with digoxin specific antibody

    Iron overdose
    Radioopaque tablets in stomach
    Abdominal pain, hematemsis, metabolic acidosis
    Treat with deferoxamine

    Cocaine intoxication
    Excruciating chest pain, atrophic nasal mucosa
    Complications include cardiac ischemia/infarction
    Treat with benzos, ASA, nitrates

    Diphenhydramine poisoing
    Anticholinergic symptoms, drowsiness, confusion
    Treat with physostigmine (cholinesterase inhibitor)

    Vitamin D toxicity
    Hypercalcemia , constipation , abdominal pain, weight loss, polyuria, polydispsia
    Treat by withholding calcium and vitamin D until symptoms resolve
