Membranoproliferative nephropathy

  • Facts:
    Nephrotic syndrome
    Three types (all three have low serum C3)

    Type I
    Subendothelial and mesangial deposits
    Associated with Hepatitis C, cyroglobulinemia, SLE, SBE

    Type II (dense deposit disease, DDD)
    Persistent activation of complement pathway
    IgG Abs against C3 convertase (C3 nephritic factor)

    Type III
  • History / PE:
    Proteinuria
    Hematuria
  • Diagnosis:
    "Tram-track" double-layered basement membrane
  • Treatment:
    Corticosteroids
    Cytotoxic agents
  • Complications:
    Slow progression to renal failure
  • Associated With:
    Hepatitis C
  • Differential Diagnosis:
    Low complement levels seen in SLE, postinfectious glomerulonephritis , MPGN
