Myasthenia gravis
  • Facts:
    Autoimmune destruction of post synaptic ACH receptors
  • History / PE:
    Ptosis
    Double vision
    Muscle fatigability
  • Diagnosis:
    EMG
    Acetycholine receptor Ab
    Edrophonium (tensilon) test (improves muscle weakness)
    Chest CT (looking for thymoma)
  • Treatment:
    Long-acting anticholinesterase (pyridostigmine, neostigmine)
    Plasmapheresis (acute severe crisis)
  • Complications:
    Myasthenia crisis (marked respiratory weakness)

    Thymomas
  • Differential Diagnosis:
    Organophosphate poisoning
    High doses of aminoglycosides
