Pneumocystis jiroveci pneumonia
  • Facts:
    P. jiroveci is an opportunistic pathogen
    Usually occurs when CD4+ count < 200/mL
  • History / PE:
    Dyspnea on exertion
    Fever
    Non-productive cough
    Impaired oxygenation
  • Diagnosis:
    Diffuse, bilateral interstitial infiltrates
    Diagnosed by cytology
  • Treatment:
    Bactrim X 21 days + prednisone (hypoxemia)
    Pentamidine X 14 days (if sulfa allergy)
  • Associated With:
    Immunocompromised hosts
    HIV/AIDS
  • Differential Diagnosis:
    Tuberculosis
    Histoplasmosis
    Coccidiomycosis
