Pseudotumor cerebri Idiopathic intracranial hypertension

  • Facts:
    Usually found in young, obese women
    Can mimic tumor/mass
  • History / PE:
    Daily HAs (worse in morning)
    Blurry vision
    Papilledema
    Abducent (sixth) nerve palsy
  • Diagnosis:
    CT/MRI negative for masses (may see empty sella)
    Intracranial HTN
    Shrunken ventricles (MRI)
    Increased opening pressure
  • Treatment:
    A cetazolamide (inhibits choroid plexus carbonic anhydrase)
    CSF draining
    Lumboperitoneal shunting
  • Complications:
    Permanent vision loss
  • Associated With:
    Isotrentinoin (used for acne vulgaris)
    Large doses of vitamin A
    Tetracyclines
    Steroid withdrawal
    Empty sella syndrome
  • Differential Diagnosis:
    Normal pressure hydrocephalus (enlarged ventricles)
