Tricyclic Antidepressants TCA

  • Facts:
    Desipramine (norpramin)
    Imipramine (trofranil)
    Clomipramine (anafranil)

    Nortriptyline (pamelor)
    Amitriptyline (elavil)

    Doxepin (sinequan)
  • Complications:
    TCA OD causes inhibition of cardiac fast sodium channels (QRS prolongation)

    Treat OD with sodium bicarbonate (helps correct acidosis AND narrow QRS prolongation)
  • Indications:
    Depression
    Insomnia
  • Notes:
    Overdose causes hyperthermia, seizures, hypotension, and anticholinergic effects. Decreased myocardial conduction velocity, leads to QRS prolongation; EKG is the best test to predict of the extent of TCA OD (QRS duration)
