Goodpasture's syndrome

  • Facts:
    Anti-GBH glomerulonephritis ( nephritic syndrome )
    Rapidly progressing disease with pulmonary hemorrhage
    Peak incidence is males in mid 20s
  • History / PE:
    Hemoptysis
    Dyspnea
    Possible respiratory failure
  • Diagnosis:
    Iron deficiency anemia
    Linear anti-GBM deposits (immunofluorescence)
    Hemosiderin-filled macrophages
    Pulmonary infiltrates (CXR)
  • Treatment:
    Emergency plasmapheresis
    Pulsed steroids
  • Complications:
    ESRD

    Wegener's granulomatosis (c-ANCA, no linear anti-GBM deposits)
