Anti-GBH glomerulonephritis (
nephritic syndrome
)
Rapidly progressing disease with pulmonary hemorrhage
Peak incidence is males in mid 20s
History / PE
:
Hemoptysis
Dyspnea
Possible respiratory failure
Diagnosis
:
Iron deficiency anemia
Linear anti-GBM deposits
(immunofluorescence)
Hemosiderin-filled macrophages
Pulmonary infiltrates (CXR)
Treatment
:
Emergency plasmapheresis
Pulsed steroids
Complications
:
ESRD
