RTA type IV Distal, hyperkalemic

  • Facts:
    Aldosterone deficiency/resistance
    Leading to defects in sodium absorption and hydrogen/potassium excretion
  • Diagnosis:
    Hyperkalemia , hyperchloremic acidosis
    Non-ionic gap metabolic acidosis
  • Treatment:
    Furosemide
    Mineralocorticoid +/- glucocorticoid replacement
    Low potassum diet in patients with aldosterone deficiency

    Renal tubular acidosis
  • Associated With:
    1) Diabetes mellitus
    2) Obstructive uropathy and renal disease (in children)
    3) Primary aldosterone deficiency
    4) Hyporeninemic hypoaldosteronism (eg. kidney disease, ACEIs, NSAIDs )
    5) Drugs (eg. amiloride, spironolactone , heparin )
    6) Pseudohypoaldosteronism
