Serum ascites albumin gradient SAAG score

  • Facts:
    Used to distinguish between portal hypertension and other causes of ascites

    SAAG score = serum albumin - ascites albumin
  • Diagnosis:
    SAAG < 1.1 (exudative protein leakage)

    1) Malignancy
    2) Tuberculosis
    3) Pancreatitis
    4) Nephrotic syndrome

    SAAG > 1.1 (imbalance of hydrostatic and oncotic pressures)

    1) Cirrhosis (portal HTN)
    2) Hepatic metastasis
    3) Budd-Chiari syndrome
    4) Cardiac disease
    5) Myxedema
